AGREEMENTS & RELEASES
Please read and acknowledge each of the following agreements. All four must be accepted in order to submit your application.
1. Eligibility & Participation Agreement
By submitting this application, the applicant (team representative) confirms that all players listed are at least 18 years of age, are legally eligible to participate in the United States, and are not under any suspension, ban, or disqualification by any recognized soccer governing body. The applicant confirms that the team has the legal authority to enter this agreement on behalf of all listed players and staff. Fútbol Fest USA LLC ("Fútbol Fest USA") reserves the right to verify eligibility and disqualify any team or player found to be ineligible at any point before or during the tournament. Acceptance of a team application does not guarantee a spot in the tournament until an entry fee has been received and confirmed in writing by Fútbol Fest USA. Fútbol Fest USA reserves the right to modify the tournament format, schedule, venue, or prize structure for reasons including but not limited to weather, safety, regulatory requirements, or force majeure events. All participants agree to abide by the official Fútbol Fest USA rules and code of conduct as published on futbolfestusa.com. Violations may result in disqualification without refund.

2. Liability Waiver & Release
In consideration of being permitted to participate in Fútbol Fest USA 2027 (the "Event"), the applicant, on behalf of themselves and all team members, coaches, staff, and associated personnel, hereby releases, waives, and discharges Fútbol Fest USA LLC, its officers, directors, employees, agents, sponsors, partners, venue operators, and volunteers (collectively, "Released Parties") from any and all claims, liabilities, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury - including death - that may be sustained while participating in or traveling to and from the Event. This waiver includes but is not limited to injuries caused by the negligence of the Released Parties. Participants acknowledge that soccer is a contact sport involving inherent risks of physical injury and agree to assume all such risks. This release is binding on all members of the participating team and their legal representatives, heirs, and assigns. Fútbol Fest USA LLC is not responsible for lost, stolen, or damaged personal property at any Event venue. All medical decisions during the Event are the sole responsibility of each participant and their personal physician. Participation in the Event does not create an employment or contractor relationship between participants and Fútbol Fest USA LLC.

3. Media & Publicity Release
The applicant, on behalf of all team members and associated personnel, grants Fútbol Fest USA LLC and its authorized media partners - including but not limited to Telemundo - a perpetual, royalty-free, worldwide license to photograph, film, record, and broadcast all participants in connection with Fútbol Fest USA 2027. This includes use of participants' names, images, likenesses, voices, and performance footage in any medium now known or hereafter developed, for purposes including but not limited to: live and delayed broadcast, streaming, social media, editorial coverage, marketing, sponsorship materials, and archival use. No compensation will be owed for any such use. Participants will not receive advance notice of specific uses. This release extends to any third-party media organizations credentialed by Fútbol Fest USA. If a participant has specific objections related to image use, they must submit a written request to info@futbolfestusa.com no later than 30 days prior to the start of the Event. Fútbol Fest USA LLC makes no guarantee that such requests will be honored.
I grant Fútbol Fest USA LLC and its media partners the rights described in the Media & Publicity Release on behalf of my team.

4. Code of Conduct & Sportsmanship Agreement
All participants - including players, coaches, managers, and team staff - agree to uphold the highest standards of sportsmanship and professional conduct throughout their participation in Fútbol Fest USA 2027. Prohibited conduct includes but is not limited to: physical violence or threatening behavior toward officials, opponents, spectators, or event staff; use of discriminatory, racist, homophobic, or otherwise hateful language; intentional damage to venue property; possession or use of illegal substances on Event grounds; failure to comply with the directions of Fútbol Fest USA officials, referees, or venue security. Fútbol Fest USA LLC reserves the right to immediately eject any player, coach, or staff member who violates this Code of Conduct, and to disqualify the associated team from the tournament without refund. Fútbol Fest USA LLC further reserves the right to report serious violations to relevant law enforcement or soccer governing bodies. The team representative signing this application accepts responsibility for communicating and enforcing these standards within their team.

Submission of this form constitutes an electronic signature binding under applicable law.
